Team Evaluation of Cooperative Skills

These evaluation forms are designed for teams in Engineering Graphic 100 classes. At the conclusion of each design project, students on each team complete this evaluation and submit it to their instructor. All evaluations will be confidential and only read by the instructor.

SCORE: For each member of your team, including yourself, rate ability to function effectively as a member of the team on a scale of 0 to100, where 0 is a non-participant and 100 is fully cooperative and participatory.

BONUS: Your design team was given $5,250 as an end-of-the-year bonus for the successful completion of the project. How would you like to see the funds distributed among your team members including yourself? Make a note of your reasons in the space below the name if you wish.
